STREET AND HOUSE TO HOUSE COLLECTIONS (PRESCRIBED FORMS) REGULATIONS, 1962 I, DANIEL COSTIGAN, Commissioner of the Garda Síochána, in exercise of the powers conferred on me by section 28 of the Street and House to House Collections Act, 1962 (No. 13 of 1962), and with the consent of the Minister for Justice, hereby make the following regulations : 1. These Regulations may be cited as the Street and House to House Collections (Prescribed Forms) Regulations, 1962. 2. The form set out in Part I of the Schedule to these Regulations is hereby prescribed as the form of application for a collection permit. 3. The form set out in Part II of the Schedule to these Regulations is hereby prescribed as the form of a collection permit. 4. The form set out in Part III of the Schedule to these Regulations is hereby prescribed as the form of revocation of a collection permit. 5. The form set out in Part IV of the Schedule to these Regulations is hereby prescribed as the form of a collector's authorisation.
