VALSTYBINĖS AUGALŲ APSAUGOS TARNYBOS VIRŠININKO Į S A K Y M A S DĖL NOTIFIKACIJOS APIE AUGALUS, AUGALINIUS PRODUKTUS, KITUS OBJEKTUS AR KENKSMINGUS ORGANIZMUS, KELIANČIUS FITOSANITARINĮ PAVOJŲ, TVARKOS 2004 m. balandžio 29 d. Nr. 67 Vilnius Vadovaudamasis Europos Komisijos direktyva 94/3/EEB „Dėl sulaikytų krovinių ar kenksmingų organizmų iš trečiųjų šalių, keliančių fitosanitarinį pavojų, paskelbimo procedūros patvirtinimo“ ir vadovaujantis EPPO rekomendacijomis: 1. Tvirtinu Notifikacijos apie augalus, augalinius produktus, kitus objektus ar kenksmingus organizmus, keliančius fitosanitarinį pavojų, formą (pridedama). 2. Įpareigoju: 2.1. regioninių augalų apsaugos ir karantino bei pasienio fitosanitarijos punktų darbuotojus nedelsiant pranešti apie užkrėstus, grąžintus, sunaikinimui skirtus augalus, augalinius produktus ir kitus objektus Augalų karantino skyriaus vedėjai L. Nečajevai; 2.2. Augalų karantino skyriaus vedėją L. Nečajevą informuoti šalį eksportuotoją, EPPO, Europos Komisiją ir ES šalių narių atsakingus asmenis apie sulaikytus krovinius pagal patvirtintą notifikacijos formą; 2.3. Augalų karantino skyrių registruoti notifikacijas, suteikiant numerį, kurį sudaro ISO Lietuvos kodas „LT“, notifikacijos metai bei individualus notifikacijos numeris iš keturių skaitmenų. 3. Pripažįstu netekusiu galios 2001 m. kovo 26 d. Valstybinės augalų apsaugos tarnybos viršininko įsakymą Nr. 42 „Dėl sulaikytų užkrėstų, grąžintų, sunaikinimui skirtų krovinių pranešimo formos patvirtinimo“.
